Supported Living

Ministry of Health Supported Independent
Living Service

Supported Living Services are designed to support individuals with as little or as much support as they require to maintain their independent living in the community. This service is provided to people who live in their own home, alone or with their family  or to people who wish to flat with others.

This service is based on the premise of Inter-dependence: this means we recognise that most of us require some level of support, be it social, physical or emotional at some stage in our lives and that the best support reflects this and allows people to maintain their independence because of it.

SILC is committed to delivering a holistic service to people on a Supported Living contract which ensures people are able to maintain the lifestyles they choose.

Who can access our services?

People who have an intellectual, physical or acquired disability and people who a have sensory challenge, who have been referred to us by SupportNet. All referrals to Supported Living Services come via Support Net who carry out a Needs Assessment to determine the level of support  you require.

Often you will have come and met us to see if we “fit” together philosophically to provide your service even before the referral is made to us. If you have not met with us we will be sure to meet you before we accept your referral to ensure we do “fit” well together.

This referral is then discussed  with Support Net to confirm if we are able provide the service to you.

Once a referral is accepted, we will meet with you to discuss when your service will begin and ask you to assist us to write your Service Plan, identifying the support you require and the aspirations you have for the upcoming few months.

For more detailed information about the Supported Living contract please review the Supported Living brochure.

"An individual strand of silk is very fragile, and easily overlooked. However, when combined with other strands of silk, it gains strength and the material is able to be manufactured into a garment that can have great value and beauty. The silk thread mirrors what can be achieved in the lives of the people we support." ©
